Launch of the "community 360": a support system for people with disabilities

The "Community 360" is being set up to facilitate information for people with disabilities and their caregivers, by breaking the isolation between the various players in the disability sector. 

The Health Agency of Guadeloupe, Saint-Martin and Saint-Barthélemy supports the ALEFPA (secular association for training, prevention and autonomy) in this implementation. Knowledge of the different systems of access to care for fragile people with disabilities is a major concern. This is all the more important in the context of the Covid pandemic which has severely affected the territory.

To improve this knowledge and share it, associations of people with disabilities, associations of caregivers, ALEFPA and ARS jointly initiated, on May 18, 2021, the cooperative approach of “Community 360”.

The "Community 360" is a new way of cooperating between establishments for the care of people with disabilities. This approach aims to advise and find a support solution for disabled people in difficulty, by mobilizing specialists in the area in synergy: associations of people, medico-social establishments and services, hospitals, health professionals, public services, etc. L he objective is to pool their resources to offer support solutions adapted to each individual.

Disabled people in difficulty as well as their family caregivers can contact for free the toll free number: 0 800 360 360 to ask for help, support or start a support process. They can also go to the CALLHANDI971 website to find all the information on daily life, schooling, transport, etc.

The deployment of the “360 Community” and the creation of the toll-free number extend and complete the “Supported response for all” approach. These devices will help improve the coordination of health professionals on complex needs, for the benefit of people in fragile situations in Guadeloupe and the Northern Islands.
